Registers that predate the advent of the National Health Service in 1948 are grouped into runs of separately maintained civil (ADA-01 and ADA-02) and medical registers (ADA-03 to ADA-07 inclusive). Between 1907 and 1948, medical registers (those that contained diagnostic information concerning patients) and civil registers (those that did not) were kept separate by Lunacy Commission rules isued on 31 October 1906.

The format of admission registers changed several times between 1683 and 1973. ARA-01 begins by listing patients in residence on 1 May 1683 before recording subsequent admissions. Volumes ARA-01 to ARA-09 give only brief details of admissions; fuller details are provided in volumes ARA-10 onwards. The dates of ARA-10 to ARA-19 overlap those of ARA-01 to ARA-09, so that the admission entries in these volumes are duplicated. The dates of ARA-22 and ARA-23 also overlap. ARA-23 is the first in a run of seventeen admission registers (also known as Commissioners' Books) which were produced to comply with Lunacy Commission guidelines on admission entries (and also include details of discharge), and much of the information in it is duplicated in ARA-22. Duplication was discontinued thereafter. Admission registers appear to have been discontinued altogether after September 1902 (ARA-39), and were resumed (in a form which recorded brief particulars only) upon the advent of the National Health Service in July 1948 (ARA-40 onwards). Registers containing fuller patient information survive for admissions from 1962 to 1971 (ARA-50) and from 1964 to 1973 (ARA-51 and ARA-52). After 1973, admission registers appear to have been discontinued (although admission and discharge registers continued to be kept until 1991).

Entries in each volume are arranged chronologically by date of admission, but the volumes are not sequential, i.e. the covering dates of each volume overlap with the next.

The Lunacy Act of 1890 permitted registered mental hospitals such as Bethlem Royal to admit uncertified patients as voluntary boarders. The Mental Treatment Act extended this permission to all hospitals.

Volumes ARN-01 to ARN-06 document male patient admissions from 1928 to 1939, volumes ARN-07 to ARN-12 document female patient admissions from 1928 to 1939, volumes ARN-13 to ARN-22 document wartime patient admissions from 1939 to 1945, and volumes ARN-23 to ARN-26 document patient admissions from 1962-1970. The wartime registers appear to exclude 'effort syndrome' patients, whose details were entered into registers in series ADP.

Volumes ARO-01 to ARO-04 document male patient admissions from 1922 to 1927, and volumes ARO-05 and ARO-06 document female patient admissions from 1922 to 1927.

BAP-001 to BAP-147 contain admission bonds, certificates and other formal admission papers only, whereas BAP-148 to BAP-295 contain files with correspondence on non-medical matters as well as formal admission papers. These files are arranged by number in two successive sequences, the first of which (contained in BAP-148 to BAP-193) is indexed in AIB-01, and the second of which (contained in BAP-194 to BAP-295) is indexed in AIB-03 and AIB-04. Most of the files numbered 1-1469 (and none after 1469) in the first sequence, which ran on to number 1860, survive. Most of the files numbered 1-2399 (and only four after 2399) in the second sequence, which ran on to an unknown number above 3360, survive.

In the nineteenth century, admission papers were posted to the Steward of Bethlem Royal Hospital in advance of the weekly meeting of the Bethlem Sub-Committee. Most of those for whom admission papers were completed were admitted to the Hospital, usually seven days after the Sub-Committee had approved their admission. However, a minority were not admitted, either because the petition relating to their admission was refused by the Sub-Committee, or, in cases in which the petition was approved, because (for whatever reason) they were not brought to the hospital to be admitted. Admission papers were not numbered and filed until admission, and papers relating to those who were not admitted must have been discarded in the normal course of events. Papers of those who were not admitted in 1815-1816 and 1848 seem to have been preserved by accident amongst the papers of those who were admitted. Unlike the main run of patient admission papers, the papers dating from 1848 often include letters from petitioners, doctors and others - perhaps because letters relating to admitted patients were removed for inclusion in the relevant casebook entry, or in other files.

Tithe apportionments are the principal record of the commutation of tithes (substitution of monetary payments for payments in kind) in a parish under the Tithe Act 1836. The original apportionments for every parish in England and Wales were retained in the custody of the Tithe Commissioners and are now in the National Archives. Bethlem Hospital kept a copy of the apportionment for Wainfleet St Mary as the landowner.

Some of the casebooks dating from 1817 to 1823 contain entries for patients housed in the State Criminal Lunatic Asylum, and strictly speaking, casebook entries derive their provenance from the State Criminal Lunatic Asylum in the case of criminal patients, as Bethlem officers were paid by the state to administer and provide care for criminal lunatics.

Dr Carlos Paton Blacker (8 December 1895 - 21 April 1975) was a psychiatrist who worked at the Maudsley between 1936 and 1960. This is a collection of his work undertaken at the Maudsley, especially in regard to record making.

After the retirement of Rev. Geoffrey O'Donoghue, which coincided with the hospital's move to Monks Orchard, religious services in the chapel (details of which are recorded in these books) were led by visiting clergy.

Volumes containing resolutions of Bridewell Hospital's Court of Governors and sub-committees concerning the State Criminal Lunatic Asylum and patients therein, copies of relevant official correspondence and copies of the quarterly patient returns made by the Asylum to the Secretary of State for the Home Office. Returns were continued after the transfer of the majority of the patients to Broadmoor (completed by 1864), and the subsequent demolition of the wing of the hospital in which the criminal patients were housed, because a small number of criminal patients were kept at Bethlem Royal Hospital into the 1880s.

From 24 October 1816 to 19 October 1820, this committee appears to have had no independent existence, but seems to have been constituted simply as and when the Bethlem Sub-Committee discussed matters relating to the State Criminal Lunatic Asylum. It seems to have become known as the Criminal Lunatic Sub-Committee from 26 October 1820 onwards. Unlike series CSA, which records the interaction of the Criminal Lunatic Department with Government, volumes in this series seem to have been solely used internally.

This run of casebooks appears to be devoted to notes on long-stay patients only (as opposed to series CWC). For admissions from 1903 to 1913, separate runs of casebooks were maintained for male and female patients (CWA-01 to CWA-09 and CWA-10 to CWA-19 respectively); for admissions from 1913 to 1939, a run of casebooks was maintained for patients of both sexes (CWA-20 to CWA-34), and overlapping runs of casebooks were maintained for male patients admitted from 1930 to 1953 and female patients admitted from 1913 to 1953 (CWA-35 to CWA-36 and CWA-37 to CWA-41 respectively).

At first, separate sets of registers were maintained for male patients (1903-1913) and female patients (1907-1912). Subsequent to this, registers contained notes on both male and female patients, and appear to overlap (CWB-06 and CWB-07 cover 1917-1934, and CWB-08 covers 1918-1931).

This run of casebooks appears to be devoted to notes on short-stay patients only (as opposed to series CWA).

The format of discharge, transfer and/or death registers changed several times between 1782 and 1991. From 1907 to 1965, deaths were recorded in separate volumes (DDR-15 to DDR-18), and a run of volumes giving brief details of discharge separate from the main run of volumes was maintained from 1910 to 1962 (DDR-19 to DDR-21).

Minutes of the meeting of 1 April 1974 are unsigned. The Estate Management and Investment Committee first met on 27 September 1948. After its meeting of 1 April 1974, the task of deciding investment policy was handed over to the Finance and General Purposes Committee. It was intended that thereafter the Estate Management and Investment Committee would meet at six-monthly intervals, usually immediately after a meeting of the Finance and General Purposes Committee, solely to review investment performance. However, the papers of the Finance and General Purposes Committee give no indication that this was ever done.

Affairs concerning Bridewell's London and country estates were handled by the London Estates Sub-Committee and the Lincolnshire Estates Sub-Committee (renamed the Country Estates Sub-Committee in 1845) respectively until 1855, when these two committees were merged to form the Estates Sub-Committee. In June 1932, the name of the committee was changed to the Estates and Finance Sub-Committee.

The Management Team of Officers and its successors, seems to have inherited many of the responsibilities of the Hospital Committee, which last met on 19 February 1976, and the Policy Sub-Committee of the Medical Committee. Signed minutes of Executive Group of Officers and its successors are missing (apart from a few between 23 October 1986 and 8 June 1989), and papers of Executive are missing after October 1994.

Minutes of Bethlem (Sub-) Committee from 1709 to 1930, the Bethlem Grand Committee from 1789 to 1802, Bethlem House Committee from 1930 to 1949, and Bethlem and Maudsley Hospital Committee from 1949 to 1976. The Grand Committee, consisting of all those hospital Governors that had been elected to the (Sub-) Committee, was summoned only to approve contracts or transact business of exceptional significance. The name of the (Sub-) Committee was changed to the House Committee in 1930. A year after the advent of the National Health Service in July 1948, and the merger of Bethlem Royal Hospital with the Maudsley, most of the functions of the House Committee were taken over by the Joint Hospital's Hospital Committee (though some others became the responsibility of the Medical Committee).

The Committee was established by the Court of Governors on 10 January 1817 'for the purpose of examining witnesses and collecting information on the question of the expediency of a chaplain being appointed at Bethlem Hospital, with directions to report to the Court'. Its report was printed by H Bryder at Bridewell Hospital at the end of 1817.

This collection is held as series LSC. A printed outline of the slides O'Donoghue used in 1905, and a catalogue of the slides prepared by Kenneth Cantle (who worked as an attendant, then a male nurse, at Bethlem from 1903 to 1947, and as part-time librarian at Bethlem from 1947 to 1957) in 1937, comprises series LSB.

This committee first met on 25 January 1956, was disbanded after its meeting of 15 September 1975, and was reconstituted on 6 September 1983. Papers of meetings from 1971 to 1972 are missing.

The Special Assessment and Supervision Service was set up by the South East Thames Regional Health Authority (with which the Bethlem and Maudsley worked in close co-operation) to provide psychiatric services for patients too disordered for care on open wards, but not so disturbed as to warrant detention in maximum security hospitals. At its heart was a central clinic (later named the Denis Hill Unit), which provided residential accommodation for thirty patients, sited in the grounds of Bethlem Royal Hospital.

Books itemising and receipting salary and allowance payments made to medical and general staff and servants. Payments to attendants were made quarterly until Christmas 1894 and monthly thereafter, and payments to physicians were made quarterly until Michaelmas 1920 and monthly thereafter. Salary payments made out of the allowance provided by the Government in respect of work undertaken by Bethlem Royal Hospital staff in the State Criminal Lunatic Asylum, which was housed in a wing of the hospital from 1816 to 1864, were itemised and receipted in SB-02 and SB-04. Salaries for staff at the hospital's convalescent home at Witley, Surrey, were listed separately within these books from Christmas 1870 until the closure of the home in 1929. Volumes SB-27 to SB-33 give salary details for members of staff who left between April 1979 and March 1988 inclusive, and SB-34 to SB-41 give salary details for members of staff who were still in the Hospitals' employ as at 1 April 1988.

These magazines seemed to either fill the gap between Inter Nos and Brainwaves, or existed at the same time as both these publications. Some seem to be printed by specific departments in the SHA or Trust.

Thematic apperception testing is also known as the picture interpretation technique and is a test in which patients are shown a series of evocative yet ambiguous about which the patient is asked to tell a story.

The Planning Committee was an ad-hoc steering group of the Board of Governors responsible for planning for the development of the hospital sites and buildings, with delegated authority to deal with matters of detailed planning policy on specific projects.

A handwritten hospital magazine known as 'The Bethelem Star' was circulated at irregular intervals from 1875 to 1880. The first series of its successor, 'Under the Dome', was also handwritten. It circulated from 1889 to 1890 or 1891, and briefly prompted a rival circular entitled 'Above the Dome'. The second series of 'Under the Dome', the first to appear in printed form, and the only to survive, ran from 1892 to 1930.

'Wounded Soldiers' Day' was a social event held in the grounds of Bethlem Royal Hospital on 28 July 1916 at which some 300 wounded soldiers resident in London hospitals attended, along with hospital staff and various dignataries. A 35mm silent black and white film of the event, complete with titles written by Rev E.G. O'Donoghue, Bethlem chaplain and chronicler, was made for the hospital by a Mr Pearce of the Essenay Company, an American film company which opened a London office, headed by Harry A. Spoor, in 1909.

The Witley Treatment and Research Unit was opened in 1930 along with the rest of the new hospital at Monks Orchard. It contained an operating theatre, pharmacy and photographic darkroom as well as accommodation for hydrotherapy, psychotherapy, and dental and electrical treatment. Lantern slides, photographs and negatives, mostly relating to patients, were held here for clinical and research purposes.

An estate of nearly 3000 acres in Lincolnshire, mainly in the parish of Wainfleet St Mary, was left to Bethlem in 1733 by Edward Barkham specifically for the 'the further and better support, reliefe and maintenance of poor incurable lunaticks', and revenue from it supported the hospital's incurable department until its closure in the early twentieth century. The hospital disposed of the majority of this land in 1919.

Bethlem had possessed property in the vicinity of Charing Cross since at least 1403. (It has been suggested that insane patients were first transferred to the hospital in Bishopsgate from this property, the Stone House, towards the end of the fourteenth century.) In 1830, Bethlem was forced to give up these holdings, along with adjoining landholders, and buildings on it were subsequently demolished, so that Trafalgar Square could be laid out, and received land and buildings in Piccadilly in exchange.

Bethlem's Piccadilly estates were procured in exchange for its disbursements of land at Charing Cross in 1830.

Bethlem Hospital left its original site in Bishopsgate in 1676, but retained its property holdings there. The majority of these were sold to the Great Eastern Railway Company and the Metropolitan Railway Company between 1865 and 1870, and the last remaining holding was sold in the 1920s.
